Patient-Centered Care
Navigating the healthcare system on your own can be an overwhelming experience. PrimeCare is proud to be your “Patient-Centered Medical Home” (PCMH) where we manage care and services for you—acting as the “hub” of your healthcare. We provide you enhanced care through same-day appointments and extended hours, and help improve interaction and communication with specialists, other care facilities, and the entire health care team.
24/7 Patient Support
We make it easy to get care whenever and wherever you need it. Our offices are open Monday through Friday from 8:30am to 5pm as well as on Saturdays from 8am to 12pm. With 24/7 on-call physicians and online resources to help answer your health questions, we’re always there when you need us!
Comprehensive Health Services
We believe strongly that providing you with health education and helping you understand your health situation can contribute to better outcomes. By replacing episodes of care with whole person care, we can help coordinate your long term health. This includes care for all stages of life: acute care, chronic care, preventative services, and end of life care.
